De Quervain's tenosynovitis causes pain at the thumb side of the wrist due to inflammation of the abductor pollicis longus and extensor pollicis brevis tendons and their surrounding sheath. It is commonly triggered by repetitive thumb movements, gripping, or wringing motions, and is particularly prevalent in new parents from lifting infants.
In TCM, this condition relates to stagnation in the Lung and Large Intestine channels at the wrist, often aggravated by cold or wind invasion of the local sinews. Acupuncture treatment utilises LI-5 (Yangxi) in the anatomical snuffbox, LU-7 (Lieque) on the radial styloid, and TE-5 (Waiguan) on the dorsal wrist. Local ashi points along the inflamed tendon sheath are needled with shallow insertion to promote circulation without aggravating the confined compartment.
A 2017 study in Acupuncture in Medicine found that acupuncture significantly reduced pain and improved grip strength in De Quervain's patients, with outcomes comparable to corticosteroid injection but without the risk of tendon atrophy associated with repeated steroid use.
The condition results from overuse of the thumb tendons through repetitive gripping, pinching, wringing, or lifting. Common causes include caring for infants, prolonged smartphone use, gardening, and racquet sports. Hormonal changes during pregnancy and the postpartum period also increase susceptibility.
Acupuncture reduces inflammation within the tendon sheath by improving local microcirculation and lymphatic drainage. Needling around the radial styloid process releases adhesions and reduces swelling. Treatment typically includes six to ten sessions combined with rest, splinting, and ergonomic modifications.
